Line 7b - Agent Company Information
If someone other than an employee of the company is completing the Remittance and
Support worksheet, enter the agent’s company name that is completing the form. This
information will be used as the contact point for collection efforts and communication.
Agent Contact Name
If someone other than an employee of the company is completing the Remittance and
Support worksheet, enter the agent’s contact name that is completing the form. This
information will be used as the contact point for collection efforts and communication.
Agent Mailing Address
Enter the complete mailing address of the agent including street address, city, state, zip,
suite numbers, floor, etc. Please do not list only a P.O. Box number.
Telephone Number
Enter telephone number for the agent.
Agent Contact Email Address
Enter the agent’s email address for questions and communications.
Section 3 – Change in Company Status
Line 8 – New Company Name
If there is a change in the company name enter the new name, indicate the former name and
the effective date of the name change.
Line 9 – Business Status
If the status of your company has changed in Texas, indicate in the appropriate space either
the date the company was sold, the date the company was merged, or the date the company
was discontinued.
Line 10 - Company Sold, Merged, or New Company
If you indicated on Line 9 that the company was either sold or merged with another company,
please provide the survivor company name. If this is a first filing or the company has recently
started operations in Texas, enter the date the company began offering services in Texas.
Section 4 - Gross TUSF Assessment
Line 11 – Total Taxable Telecommunications Receipts (Reported under Chapter 151 of the
Texas Tax Code). Enter the total taxable telecommunications receipts.
Line 12 – Interstate Receipts
Line 12a. Enter the actual total taxable telecommunications services receipts that can be
identified as jurisdictionally interstate or international.
Line 12b. Enter the total taxable telecommunications services receipts that can be
identified as jurisdictionally interstate or international using a safe harbor percentage.
Line 12c. Enter the sum of Lines 12a and 12b.
