Line 16 – Total High Cost Support
Texas High Cost Universal Service Plan (THCUSP)
This section applies to telecommunications carriers that have been designated as ETPs by the
Commission pursuant to PUC Substantive Rule 26.417 and provide service in high cost
areas. ETPs requesting support under this plan must provide supporting documentation for
each wire center as reported in NECA’s Tariff #4. For additional information, see Substantive
Rule 26.403.
Line 16a - Enter the Base High Cost Support amount for all wire centers. Provide supporting
calculations identifying the name of the ILEC, CLLI code, number of lines, base support
amount per line and resulting base support associated with the TUSF high-cost disbursement
request for each ILEC serving territory.
Line 16b – Enter the total amount of the UNE USF support per lines serviced by UNE
purchase. (This is the total from Section 8, Line F for CLECs and Section 8, Line G for
ILECs.)
Line 16c – Enter the total amount of federal USF high-cost support received by your
company that is required to be deducted from the base support pursuant to PUC S
. R. §

26.403(e)(3)(B). If you provide service in the incumbent local exchange service territories of
AT&T, Verizon, Central Telephone Company of Texas and United Telephone Company of
Texas and/or Valor Telecommunications of Texas and receive federal USF high-cost support
specifically associated with these service territories, such federal USF high-cost support must
be reported and deducted from the base support. The federal USF high-cost support that must
be deducted is that associated with the federal high-cost loop, safety valve and safety net
programs. Separately identify federal USF high-cost support for each ILEC on the supporting
documentation.
Line 16d - Enter the amount of Line 16a - 16b - 16c
Small and Rural Incumbent Local Exchange Carrier (ILEC) Universal Service Plan
(SRIUSP)
This section applies to small ILECs, rural ILECs and telecommunications providers other
than small or rural ILECs that provide service in small and rural study areas that have been
designated ETPs by the commission pursuant to PUC Substantive Rule 26.417.
Enter the monthly total number of eligible lines served by the ETP in its study area pursuant
to PUC Substantive Rule 26.404(g)(1), and the monthly per-line amount of support pursuant
to Substantive Rule 26.404(e) as determined by the Commission. Attach supporting
documentation if serving more than one study area showing each study area and the number
of customers in each.
Line 17 – Total Small & Rural ILEC Support
Enter the total Eligible lines served in box provided. Multiply the number of eligible lines
served by the ETP in its study area by the monthly per-line amount of support as determined
