19d ILEC’s selling Wholesale Lifeline Discounts to ETP’s need to provide an excel file
listing company sold to, number of lines, amount of discount, total dollar sold. If no
discounts were sold put an X in the box.
Line 20 – Total Lifeline Settlement Agreement Order 34723 SUBT. R. § 26.412 (f)(1)(f)
This section applies to designated eligible telecommunications providers (ETPs) as defined
by PUC Substantive Rule 26.417 and to resale eligible telecommunication providers (R-
ETPs) as defined by PUC Substantive Rule 26.419, who have qualified Lifeline subscribers
that participate in this program. Order 34723 Subt. R. §26.412 (f)(1)(f) Settlement
agreement allows additional lifeline support by territory beginning with January 2009 data
month.
There are 2 sections in the Total Lifeline Settlement Agreement. Posted Rate is for
customers that are using the rate approved. The Variable Rate section is to be used for
customer counts and amounts that have different rates for their customers. Total Territory is
the total of the customer counts for Posted Rate and Variable Rates, the Total Amount is the
total Posted Rate and Variable Rate amounts.
20 a – List the number of qualified lifeline customers in the AT&T Territory and multiply by
the settlement agreement rate for this territory. Enter in Posted Rate and /or Variable Rates
and total in Total Territory.
20 b – List the number of qualified lifeline customers in the Embarq Territory – Central and
multiply by the settlement agreement rate for this territory.
20 c – List the number of qualified lifeline customers in the Embarq Territory - United and
multiply by the settlement agreement rate for this territory.
20 d – List the number of qualified lifeline customers in the Verizon Territory and multiply
that by the settlement agreement rate for this territory.
20 e – List the number of qualified lifeline customers in the Windstream SW Territory and
multiply by the settlement agreement rate for this territory.
20 f – Total the lifeline Customers and the sum of the credits 20a-20e.
Line 21 – Total Tel-Assistance Support
Enter the monthly number of qualified low-income customers to whom Lifeline Service was
provided through Tel-Assistance. Enter on Line 21 the total dollar amount of the discount
associated with the monthly number of qualified low-income customers reported and not
recovered by the Federal Lifeline Program. Attach supporting documentation listing each
study area, the number of associated Tel-Assistance Lifeline customers in each study area,
and the dollar amount provided per customer for each study area.
