Line 18 – 21 Total Lifeline Support
Lifeline Program
This section applies to designated eligible telecommunications providers (ETPs) as defined
by PUC Substantive Rule 26.417 and to resale eligible telecommunication providers (R-
ETPs) as defined by PUC Substantive Rule 26.419, who have qualified Lifeline subscribers
that participate in this program. For additional information, see Substantive Rule 26.412.
Tel-Assistance Support should not be included in Line 18, it should be listed on Line 21.
Line 18 – Total Lifeline Support (Non-TSR Companies)
18a Enter the monthly number of qualified low-income customers to whom Lifeline Service
was provided. Enter on Line 18a the total dollar amount of the discount associated with the
monthly number of qualified low-income customers reported and not recovered by the
Federal Lifeline Program. The maximum amount of support per customer is $3.50. This
does not include partial credits.
18b Enter the total adjusted Lifeline Support or true-ups and adjustments made this data
month. Any adjustments that are not full month credits must be listed on this line.
18c Enter the total of Lines 18a + 18b for this months total lifeline support.
18d ETPs purchasing Wholesale Lifeline Discounts from ILECs need to list the total number
of discounted lines purchased and the total amount charged from the ILEC for these lines. If
no lines are purchased put an X in the box.
18e ILEC’s selling Wholesale Lifeline Discounts to ETP’s need to provide an excel file
listing company sold to, number of lines, amount of discount, total dollar sold. If no
discounts were sold put an X in the box.
Line 19 – Total Lifeline Support (TSR Companies Only)
Enter the monthly number of qualified low-income customers to whom Lifeline Service was
provided through Total Service Resale (TSR). A R-ETP designated pursuant to PUC
Substantive Rule 26.419 shall not receive state or federal Lifeline support if the Lifeline
Service was purchased as a wholesale Lifeline offering from the ILEC. Lifeline Service
purchased as a wholesale offering from the ILEC already includes the Lifeline discount
and is therefore not eligible to receive an additional discount. Resale ETPs must attach a
supporting document listing each study area or wire center served and the number of
associated Lifeline customers in each study area or wire center and the support requested for
each study area or wire center.
