Line 13 - Intrastate Receipts
Enter your intrastate telecommunications receipts as defined in Public Utility Commission of
Texas Substantive Rule 26.420(f).
Line 13a. Enter your intrastate receipts that can be calculated on an actual basis. This line
can not be left blank.
Line 13b. If you need to use a safe harbor percentage to identify the intrastate portion of
total telecommunications receipts, enter the total amount of your receipts to which a safe
harbor percentage will be applied.
Line 13c. Enter the PUCT Docket Number granting your safe harbor waiver. A docket
number must be entered in the box to use the safe harbor percentage. List your approved
safe harbor percentage in Line 13c.
Line 13d. Enter the result of multiplying Line 13b times the “safe harbor” percentage
approved by the PUCT in line 13c.
Line 13e. Enter the sum of Lines 13a and 13d.
Receipts in this section should be reported in dollars and cents rounded to two decimal
places using half rounding. For example: $488.885 would be rounded to $488.89 and
$488.884 would be rounded to $488.88.
Line 14 - TUSF Assessment Rate
Effective January 1, 2009 the Texas Universal Service Fund assessment rate is 3.4%.
