program’s long-term outcomes/impacts (e.g., high school completion, college readiness);
4. Emphasize the outcomes that the proposed evaluation will measure; and
5. Include only information that is directly related to the theory of change. Keep the logic model simple to
minimize attention to things that cannot be controlled.
The logic model should ultimately be used to guide the collection of data to inform an assessment of program
effectiveness. The logic model should emphasize details of the program that directly relate to the core aspects
of the evaluation.
